1. To register a child or young person on the disabled children and young person's register you will need to have a My Council account.
  
  A My Council account will allow you to keep the details we hold on the register up to date.
  
  With your account you can also access personalised services and save time using our website again, as your details including your name and address will be pre-filled making forms easier to complete.

Before you start
1. We are required by law to keep a voluntary register. This is of children and young people from birth to 25 years old who live in Stockton-on-Tees and have a disability or special educational need.
  
  We use anonymous information held on the register to plan future services for children with disabilities in Stockton-on-Tees.
  
  Under the Equality Act 2010 a disability is defined as ‘a physical or mental impairment that has a substantial and long-term negative effect on your ability to do normal daily activities.
What happens to my information?
