LiveWell Hub

The LiveWell Hub is the base for the Stockton-on-Tees Adult Carers Service, Dementia Service and Shared Lives Service. It is a community-based service that provides dementia information, support and training for carers and anyone living with dementia in Stockton-on-Tees.

The Hub environment has been designed to better support people with dementia and offers a calm space where you can access:

  • up-to-date information about dementia 
  • information about the services, support groups and social activities available locally 
  • memory service appointments
  • dementia training

This includes people with dementia, their families, their carers and anyone who would like to learn more about the condition.

Dementia Advisor Service

We can provide specialist information and advice about dementia, local support services, planning for the future and strategies for dealing with common challenges.  

Advisors work with people from the early stages of diagnosis. They make sure people with dementia and their families have the knowledge, skills and confidence needed to help with health, care and self-management. 

Key areas of advice are on:

  • understanding dementia
  • getting a diagnosis and what to expect
  • where to access information when needed locally
  • accessing social opportunities and peer support networks
  • accessing benefits and other entitlements
  • carers support
  • maintaining your emotional wellbeing
  • staying safe at home with assistive technology
  • planning for the future
  • managing dementia related changes in behaviour
  • managing dementia related changes in communication
  • managing daily living activities
  • maximising health
  • accessing health and social services
  • care homes
  • getting involved in dementia related research 

 

Meet our Dementia Advisors

Tammy Rhodes

Tammy's role is to offer advice and support to people from pre-diagnosis to people who are recently diagnosed or further into their journeys of dementia. Tammy will offer advice and support to the families and carers of the people living with dementia and this can be ongoing as and when they require it as the dementia progresses through the different stages.

Tammy feels very passionate about supporting people who live with dementia and their carers. She has lived experience of dementia as well as experience of working with lots of people over the years who live with dementia.

Beverley Ward

Beverley's role is to provide support and advice, or simply to listen to people from all walks of life whose loved ones are navigating their dementia journey. Everyone's journey is unique, and Beverley hopes to make a difference to their lives by supporting, advising, listening, signposting and empowering individuals who access our service.
