We aim to provide services which will ensure the well-being, independence and safety for older people and their carers.

We organise and provide a range of services to support older people living in the Borough and their carers.

Dementia Services

In 2012 we formed the North of Tees Dementia Collaborative. This is a joint programme between the following health and social care organisations in Stockton and Hartlepool.

  • Stockton and Hartlepool Clinical Commissioning Group
  • North Tees and Hartlepool NHS Trust
  • Stockton Borough Council
  • Hartlepool Borough Council
  • Tees, Esk and Wear Valleys NHS Foundation Trust

The aim of the North of Tees Dementia Collaborative is to deliver large-scale change to improve services for people with dementia in Stockton-on-Tees and Hartlepool. It had been endorsed by the Chief Executive of the statutory organisations involved.


The Herbert Protocol

Do you care for someone who has dementia and worry that they might go missing? The Herbert Protocol is here to help find them if they do.

Nothing is more worrying or distressing than when a loved one or friend goes missing or doesn't return home when expected.

For people living with or caring for someone with dementia, this may be quite common. The Herbert Protocol is a national scheme adopted by Cleveland Police and other police services across the country.

It encourages carers, families, friends or neighbours, to hold information about the person with dementia that can help the police find them if they do go missing.

The basis of the scheme is for vital information about the person such as medication, description, photograph, significant places in the person's life and their daily routine, to be recorded on a form.

Stockton-on-Tees Over 50's Forum

The Stockton-on-Tees Over 50’s Forum is open to any resident living in the Borough over the age of 50. Each month there is a different guest speaker on topics related to being an older person in the Borough, for example Warm Homes Healthy People and Health Issues in Older People. The forum are actively looking for new participants. The Stockton Information Directory provides information and advice on a range of services and opportunities for adults across the borough of Stockton. The wide range of services include information and advice on where local groups or leisure activities are taking place, what help you can get to remain in your own home as well as housing and care home information, and much more.
