Stockton Council's 'Oasis' offers after-school and day-care short-breaks for children and young people (aged 5 (first September after the child's 5th birthday) to 17 years) with a learning disability.
To access this service you will need a referral from a social worker, who will have completed an assessment of need and presented your case to a resource panel who will agree how we can best help to meet the needs of your child.
Once we receive the referral a member of staff will undertake a home visit, where we will obtain additional information that will help us to understand what level of care your child will need whilst in our care.
We will give you a list showing the dates that your child can attend OASIS (six months at a time). This is a free service but we do ask for a small contribution to help towards the cost of the activities your child may participate in.
After School sessions
Children are admitted from school by our 'educational transport' service.
Please collect your child at 7:00pm.
Please bring your child at 10:00am and a time to collect your child will be agreed prior to your child accessing the service.
Please bring your child to the building and collect them from the building at the end of the session. This allows staff to discuss plans for the day and to discuss any issues relating to your child.
Please tell us if another person will collect your child at the end of the day.
Please tell us if your child is not able to attend a session as we will try to allocate a place to another child.
Please provide a packed lunch for your child for both after school care and day care sessions. Drinks and snacks are available throughout the day. Staff eat alongside the children and help where needed.
Children should wear appropriate clothing and footwear for indoor/outdoor use - we sometimes decide to go out for a walk so please ensure your child brings a coat. Belongings should be clearly marked with your child's name. You will be informed in advance if your child requires swimming equipment.
Please supply any toiletries that your child requires, such as continence pads, sanitary wear or baby wipes.
It is important that you ensure staff are aware of any medical conditions or allergies that your child has. If your child requires medication during their time at OASIS you must have these signed in by a member of staff on arrival. Medication must be in a container that has a pharmacy label clearly stating the child's name and prescribed dosage. Parents will be required to sign a form detailing the child's medication. If there is any medication left over from your child's session you will be required to collect it as we are unable to store it.
If your child appears unwell we will contact you and ask that you collect them. Parents will be required to sign a form to give permission for staff to seek medical advice in an emergency (any decision to give treatment will be made by the emergency health services).
We also offer a school holiday care programme (inclusive holiday programme) for children and young people with a learning disability (aged 3 to 17 years).
Your child will be allocated a venue to attend, according on their age and ability.
Booking forms are available from OASIS. Completed forms should be submitted to OASIS and bookings received before the programme starts.
Parents are responsible for transporting children to and from the allocated venue. There is a charge for each day session and you are required to pay for all of the booked sessions your child is due to attend at the first session.
