Multi-Disciplinary Service

A new integrated Health and Social Care team has been developed to provide short term early intervention and prevention to help people remain safely in their own home.

The new Multi-Disciplinary Service (MDS) is provided by Stockton Borough Council and North Tees and Hartlepool NHS Foundation Trusts. The longer term goal is to prevent avoidable hospital admissions and referral into long term social care.

The service is available to people:

  • Aged 65 years and over;
  • Living in the Borough of Stockton; and
  • Who are not currently under the care of a social care professional.

Those benefiting from the service will receive a comprehensive wellbeing assessment, and an integrated support plan will be developed with a Well-being Facilitator. The support plan may include a referral to other health, social care and voluntary sector services, co-ordinated by the Well-being Facilitator.

Clients do not need a Care Act Assessment and any support / intervention given during the first 6 weeks is free.

Referrals can be made via SPA for health and First Contact Adults for Social Care.

Falls Service

The new falls service in Stockton is based at Tithebarn House and is a part of the Multi-Disciplinary Service (MDS). The service is provided by two Falls Assessment Coordinators.

The aim of the service is to provide education, assessment and early intervention to help prevent people from falling.

To qualify for a referral to the Falls Service, clients must be aged 65 and over with one or more of the following:

  • More than 1 falls in 12 month period
  • Presentation for medical advice following a fall

Exclusion Criteria

  • Under 65 years
  • Falls caused by alcohol or drugs
  • Clients with a diagnosis of impaired cognition who are unable to co-operate with assessment and treatment (Client lacks capacity/ under DOLS)
  • Terminally ill (End of Life Clients)
  • Acute illness
  • Falls as a result of a neurological condition (please liaise with the relevant health care professional / neurophysiotherapy who can recommend more condition specific interventions)
  • Mechanical falls due to slips and trips (e.g environmental hazards, rugs, trailing wires)
