If you think you may need care and support and need a Care and support Assessment. You can do this by getting in touch with the First Contact team.
It is their job to gather information to make an assessment. The team can take information from you, a relative, a friend, or a professional, providing permission has been obtained from the person concerned.
They deal with requests for services for people with visual impairment, people with a physical disability, people with a learning disability, elderly people and for children and families. They can also advise if there is a more suitable agency to deal with a query and provide relevant contact details for those agencies.
All non-urgent professional referrals to Adult Social Care (Social Work/Occupational Therapy/Multi-Disciplinary Service/Sensory Support) should be made only by completing the Adult Social Care Referral Form.

Alternatively, you can find out if you are eligible for care and support by using the Stockton Eligibility Indicator, provided by Stockton Information Directory (for Adults).
The eligibility indicator is an intelligent interactive tool for adults to quickly and anonymously establish their likelihood of eligibility for care and support from the local authority. The user is presented with a result of likelihood for eligibility for care and support along with whether or not they are likely to have to pay for some or all of the support they need. A full assessment would still be required following completion of the Stockton Eligibility Indicator.
