Who is the Home Care Service available to?

Adults, 18 years and above who live in the Borough of Stockton-on-Tees.

How do I access the Home Care Service?

If you agree, you may be referred to the service by a number of care professionals, yourself, a friend or family member.

How does the Home Care Service work?

The appropriate Health or Social Care professional will visit you to determine your needs and develop an individual plan of the care you need.

What help will I be given by the Home Care Service?

This will be agreed between yourself, your Social Worker, and your carer if necessary, following the assessment of your needs. If you need personal care for the first time you will receive this through the Intermediate Home Care Service for up to six weeks. During this time, health and social carers will work with you to improve your independence and build your confidence. A care plan will be drawn up outlining the help you need, when you need it and who will provide it.

Will I have the same Home Care Service carer all the time?

During the time you are cared for by the Intermediate Care team you will have one or two carers only. If you require higher levels of longer term care your "Care Package" will be transferred to the Local Authority Home Care Service and will be undertaken by a small team. If a Carer is absent for any reason and an alternative carer replaced him/her we will contact your prior to the change to advise you.

Will the Home Care Service carers know what assistance I need?

Yes. Your care plan will be in your home for the Carer to read. You will also have an action plan which will describe in more detail how each task is to be carried out.

Will I be able to have my Home Care Service visits at the time I want?

We will always try to fit calls in to suit each clients' preference but this is not always possible, especially at main meal times. When the service is set up we will agree to time band with you which give the carer a degree of flexibility in which to meet your particular requirements.

Will I have to pay for the Home Care Service?

The service provided by the Intermediate Home Care Team is free. If you require longer term care your Social Worker will carry out a financial assessment of your means. This will determine what your contribution will be towards the service. This will be discussed with you before the service is transferred.
