Registering a death

What documents do I need to register a death?

You must present either a medical certificate issued by a doctor, or a Coroner's certificate issued by the Coroner - this will be given to you in a sealed envelope. If the Coroner is issuing a certificate, they may fax it direct to our office - in that case you would not be required to collect the original from the Coroner. It would be helpful if you bring with you the deceased's medical card, if available.
