Registering a birth

What information is required to register a birth?

  • Date and place of birth
  • Full names and surname you have chosen for your baby
  • Whether baby is a boy or a girl
  • Full names, occupation, place and date of birth, and current address of mother
  • If mother has ever married, her maiden name
  • Full names, occupation, place and date of birth, and current address of father (if father included in birth entry)
  • If parents are married, month and year of marriage and number of children the mother has had previously
  • Baby's NHS Number, which is normally printed on either the cot card or yellow book from the hospital, or the Explaining the NHS Number leaflet
