If your home is unsuitable for your needs

I have completed a reassessment form on medical grounds. What happens now?

Tristar Homes will visit you at home and conduct a short interview. The information will be passed to the Housing Options team, enabling your application to be reassessed. All applications for considertation on the grounds of medical need are processed by the Senior Clinical Medical Officer for the local Primary Care Trust. A medical category is only given to cases where it is deemed that the medical condition of a person living in the household is such that the present home is unsuitable for them, and that a move to another home would help to alleviate the medical problem being experienced.
