If your name is already on the Housing Register or the Transfer Register, a form for a request for consideration on medical grounds can be obtained from the Housing Options Service at 16 Church Road, Stockton (01642 393872) or from Tristar Homes, 16 Church Road, Stockton (01642 393827), or any local housing office.
However, if your name is not already on the Housing Register or Transfer Register, you will need to fill out a Housing Application Form and a Medical Form.
