Active Health is a twelve-week progressive exercise programme. Activities can be taken at Stockton Sports Centre, Splash, Billingham Forum, Thornaby Pavilion, Thornaby Swimming Pool or Ragworth Neighbourhood Centre, Arc, Blakeston School and Northfield Sportsdrome.
The scheme is a service provided by Stockton Borough Council in conjunction with local GP's and other Health Practitioners to help manage and control your medical condition. It is a great alternative way to improve your medical condition, health, lifestyle and well being.
What to expect at the initial consultation:
- Once you have a referral form you will need to book a consultation at one of the facilities listed at the bottom of the referral form. A consultation will cost you £1.80.Gym Induction is £5.00
- Bring your referral form with you. The Health and Fitness Advisor will keep the yellow copy, you will be given the white copy and your referrer should have kept the blue copy before giving it to you. Further costs for your activities will be explained during your consultation. We do heavily subsidise the programme to make it accessible to as many people as possible.
- During this 30-minutes 1:1 chat we will take some basic information from you to learn more about your likes and dislikes as well as information on your medical background.
- The Health and Fitness Advisor will outline the Active Health programme and the various physical activity options available to you and a date for you to start exercising.
- Each participant on the programme receives an information pack with all the information needed to start the scheme.
- On completion of the programme you will need to book another consutlation in which we will evaulate your progress and give you options so you can continue exercising.
- You will only be given one referral to the programme unless your medical circumstances have changed e.g. high blood pressure, heart attack - or if you have attended cardiac rehab'.
