Active Health: Exercise Referral Programme

What is it & Who is it for?

Active Health is a 12-week exercise referral programme for those who have a physical or psychological condition which can be improved through taking part in physical activity.

You will first need to speak to a medical professional e.g. doctor, nurse, Physio, health trainer etc. to discuss your medical condition and any medications you are taking. Your medical advisor will then refer you onto the programme if they feel you will benefit from it.

How to get started

Once referred onto the programme you will be given an Active Health referral form with a range of participating venues (see below) and contact numbers. You will then ring your chosen venue to book an initial consultation (a 30 minute 1:1 session to talk about you medical conditions and the appropriate activities available to you); this session is currently charged at £2.00.

You will be offered a choice of the following physical activity options depending on what each venue has available:

  • Gym Programme
  • Studio Based Exercise Class
  • Water Based Exercise Class
  • Pilates Class
  • Tai Chi Class
  • Health Walks
  • Postural Stability Class

How much does it cost?

  • Initial and End Consultation - £2.00
  • Each activity session - £2.00
  • Gym Induction - £5.80
