Cardiac Rehabilitation

"Cardiac Rehabilitation is the process by which individuals with cardiac disease, along with a multi-disciplinary team of health professionals, are encouraged and supported to achieve and maintain optimal physical and psychosocial health".

In the UK cardiac rehab is traditionally described in terms of recovery:

  • Phase 1 is the inpatient stage.
  • Phase 2 is the early discharge period.
  • Phase 3 is the clinically supervised outpatient programme.
  • Phase 4  is a clinically supervised session which leads onto a long-term maintenance of physical activity and lifestyle change

The Cardiac Rehabilitation programme is a referral programme.  If you would like to receive further information please contact Vicky Fawcett on 01642 524816 or email: vicky.fawcett@stockton.gov.uk.
