Youth & Leaving Care Service

The Youth & Leaving Care Service Team works with young people between the ages of 15 and 21 years who

• are preparing to leave care
• were looked after on their 16th birthday and who require after care services
• are 16/17 year olds who are homeless
  

Services Provided are:

• A Duty service which undertakes a needs assessment
• Information with accommodation
• Family conciliation work
• Practical and emotional support
• Leaving care services
• After care services
• Drop In Service
• Personal Advisors

Services are obtained in a number of different ways:

• By the First Contact Unit
• By the current Social Worker
• By other Agencies e.g. Housing, Health etc.
• Self referral from young people and their families
