Aims to meet the health and social care needs of local people who experience severe and enduring mental health problems and to support their carers.
Referrals are received from a variety of places but mainly the GP will refer people, following which an assessment of need is undertaken with you and your carers (where relevant). A Care Programme is then written with you and this is your agreed plan of care, co-ordinated and planned with you, carer and the care team. A Care Co-ordinator is then responsible to oversee and help deliver your care plan. The care plan is regularly reviewed with you, your carer and care team and will be changed as and when appropriate.
Clifton House
Clifton House is a Resource Centre in the community for adults who have a GP in Central Stockton.
It provides Day Services, Outpatients and Community Mental Health Team under one roof.
Ideal House
Ideal House is the base for Thornaby, Yarm and Eaglescliffe providing Community Mental Health Team, Day Services and Outpatients appointments.
Billingham and Norton
Queensway and Parkside Day Centre at Billingham are the current bases for Billingham and Norton clients.
Outpatient Clinics for this area are currently held at North Tees Hospital.
All bases are open Monday to Friday 9 to 5 pm.
