Adults Learning Disability Service
This Service brings together specialist health & social care services for adults with learning disabilities. The service consists of two teams.
- Care Management Team Adults
- Resources Team Adults
Each team comprises a range of professional support staff including doctors, nurses, social workers and therapists.
Clients who want to use the service will be assessed. The assessment will then be used to produce a Care Plan which will be reviewed on a regular basis.
The resources available include day services, respite, residential services and community support.
Alternatively services can be purchased through Direct Payments.
