The Personalisation Agenda

We are changing the way social care services will be delivered in Stockton-on-Tees through a process known as self directed support.  This transformation is part of a new vision for social care services set out in the Putting People First concordat (December 2007).  We know that people who use the services, their families and carers have a good understanding of how their illness or disability impacts on their daily life and this knowledge has helped the idea of self directed support grow in importance.

There are four areas which Stockton-on-Tees Borough Council and our partners will be focusing on to help make sure services become more personalised and to get the right results for people. These are:

Choice and control - is people directing their own support and managing the resources available to them to access services that meet their needs rather than having to fit in with the things on offer.

Self directed support and individual budgets will allow people the freedom to choose the who, when and where of any support they require.  It will also require individuals to be responsible and show that any money spent is meeting their agreed outcomes. To do this the right information and advice must be available to all, including those who will require extra support to remain safe.

Universal services - are the general support services available to everyone locally.  Things like transport, leisure, community groups, education, health, housing, community safety and access to information. Everyone must be able to access the information, advice and support they need, be able to use them effectively and be made to feel welcome.

Early intervention & prevention services - assist people who need a little help, at an early stage to remain independent. These can be things like help to manage a long-term condition, help to access better housing options,  help to safely maintain home and garden, training to get a job, or support to start taking some exercise. 

Social Capital - is all about individuals being full members of their community and making a contribution to it in a way that is meaningful for them.  This can involve things like community networks, neighbourhoods, volunteers and service user led organisations.  Everyone is entitled to the opportunity to be part of a community and experience the friendships and care that can come from families, friends and neighbours.
