Long Term Conditions

There is a North of Tees Strategy 2008-11, a shared health and social care vision and commitment to deliver quality care for those with long-term conditions. The strategy is intended to communicate a shared vision for the future management of long term conditions across the North of Tees , and provide a framework to enable relevant local groups/service providers to develop action and implementation plans necessary to realise improvement .

Locally a 2008 Action Plan is being developed.
