Adults Social Care

We provide a wide range of services for adults with care needs, including older people, people with mental health problems, people with disabilities or people who have a learning difficulty.

An assessment of need is the basis for deciding what kind, and level, of service should be provided.

Access to services is then arranged to meet individual care needs and can include:

  • Adult support and protection
  • Home care
  • Day care
  • Care home
  • Counselling and benefits advice
  • Post hospital care
  • Equipment and adaptations
  • Personal budgets

Together with health service partners, a Community Health Partnership Strategic Plan is produced.

You might also find the service offered by The National Careline of use. The National Careline is a a not for profit company offering information about care and support for older people, their carers and their families. It aims to fill the information and advice gap for older people and their relatives who are seeking to find their way through the care maze. More information can be found via www.thenationalcareline.org or by calling 0800 0699784.
