Adults Social Care
We provide a wide range of services for adults with care needs, including older people, people with mental health problems, people with disabilities or people who have a learning difficulty.
An assessment of need is the basis for deciding what kind, and level, of service should be provided.
Access to services is then arranged to meet indivual care needs and can include:
- Adult support and protection
- Home care
- Day care
- Care home
- Counselling and benefits advice
- Post hospital care
- Equipment and adaptations
- Personal budgets
Together with health service partners, a Community Health Partnership Strategic Plan is produced.
