The Disabled Childrens Team provides a social work service to disabled children and their families. The service provides social workers (who will undertake assessments of need and carers assessments and provide support and advice), and support services, in the form of overnight respite care, day care at weekends and school holidays, teatime care, and support services in the home. Social Workers also provide information, support and advice regarding Direct Payments. Services are provided to those children and young people with "permanent and substantial impairments" which means that it is a service for young people who will require support on a lifelong basis.
The team works closely with the Learning Disabilities service provided by the Tees and North East Yorks NHS Trust, with schools and the Education Service in Stockton, with paediatric services at the University Hospital at North Tees and with other agencies
